CHICAGO (CBS) –A 54-year-old Chicago Fire Department captain died Friday after helping fight a two-alarm blaze in Chicago’s Gage Park neighborhood.

Herbert Johnson, 54, incurred fatal injuries and another firefighter was injured while battling a fire at 2315 W.  50th Place that began in an attic. A “flashover” may have occurred, fire officials said.

Updated: 3 minutes ago

A Chicago firefighter has died from injuries suffered while battling a South Side fire. Two firefighters were injured in the South Side house fire, and witnesses say they saw fellow firefighters performing chest compressions on one of the injured before he was rushed away.

Another article can be found HERE and HERE including the excerpt below:

One firefighter from the fire in the 2300 block of West 50th Place was taken to Advocate Christ Medical Center in Oak Lawn for treatment and the other was taken to the University of Chicago Medical Center, both initially in serious-to-critical condition, Fire Department spokeswoman Meg Ahlheim said.

The firefighter at U. of C. died at the U. of C. emergency room after paramedics had to perform CPR on him at the scene, said Fire Department spokesman Larry Langford.

The victim at U. of C. recently was promoted to captain, Ahlheim said. The one taken to Christ Medical Center was admitted in good condition, she said.

Reports say that the LODD was a captain from the 5h District.

CFD 2-11 Mayday – 2 members injured

The Chicago Tribune just posted the following article:

Staff report6:06 p.m. CDT, November 2, 2012
Two firefighters were seriously injured in a 2-11 alarm fire on the South Side that also brought an Emergency Medical Services Plan response from fire officials this evening.

Firefighters on the scene called a Mayday response for a trapped firefighter, but the firefighter was located, and the response secured, with two firefighters injured and taken to area hospitals in serious-to-critical condition, Chicago Fire Department spokeswoman Meg Ahlheim said.

Firefighters were called to the scene at 5:15 p.m. for the fire on the 2300 block of West 50th Place. The building was a frame home, 2-1/2 stories tall and about 25 feet by 40 feet, Ahlheim said.
Four adults and four children were displaced by the house fire, she said.

The fire originally was in the attic when firefighters arrrived, and both firefighters were injured inside the building, said Ahlheim, who had no details about their injuries.

One firefighter was taken to Advocate Christ Medical Center in Oak Lawn for treatment and the other was taken to the University of Chicago Medical Center, she said.

Prospect Heights history

This from Deputy Chief Drew Smith:

Here are a few pages from an old Prospect Heights log book.  Many departments still keep a hand-written log book, but for whatever reason this practice was discontinued here by 1981.  The first working fire entry is recorded by Captain Ron Olsen who was later promoted to deputy chief. He retired from that rank and continues to serve with the PHFD as a part-time fire inspector.

Chief Olsen also made the second entry. It has additional entries by Lieutenant Warren Lippert and Captain Norm Bittner.  Lt. Lippert’s son is retired Arlington Heights firefighter Dean Lippert. Also, it appears that around November 1, 1971 is when the PHFD adopted the Code 1, 2, 3, 4 system used by many fire departments.  I believe the origin of this can be attributed to the Elk Grove Village FD.
